Pat Farmer

Pat Farmer is a Project Manager at Discern Health. She works with primary and specialty medical practices to train them in patient-centered transformation concepts and assist with evidence production. Pat’s recent work at Discern includes patient-centered medical home (PCMH) coaching for DeLeo Family Medicine in Kingston, New York, patient-centered specialty practice (PCSP) coaching at the MedStar Georgetown University Hospital Multiple Sclerosis Center, in Washington, DC, and electronic health records specialist on the Athens Neighborhood Health Center PCMH transformation project in Athens, Georgia. She assists with development of online training modules and evidence templates for the Discern Value Essentials eLearning platform.

Prior to joining Discern Health, Pat was a medical practice manager, teacher, and practicing Registered Nurse.
