Pat Farmer is a Senior Advisor at Discern Health, focused on patient-centered practice transformation. Prior to advising, Pat was employed as a Project Manager with Discern. She worked with primary and specialty medical practices to train them in transformation concepts and designed tools and templates to support practices in evidence production for National Committee for Quality Assurance (NCQA) recognition. Pat’s recent work at Discern included patient-centered specialty practice (PCSP) coaching at the MedStar Georgetown Multiple Sclerosis and Neuroimmunology Center, in Washington, DC. Pat’s other project work included research, writing, and development of training materials.
Prior to joining Discern Health, Pat was a medical practice manager, teacher, and practicing Registered Nurse.
