Discern Completes Project With Holy Cross Hospital

Discern Health recently completed a project advising Holy Cross Hospital, a community hospital in Maryland with over 400 beds, on how to reduce unplanned 30-day readmissions. Holy Cross Hospital is a part of the Trinity Health system and has approximately 35,000 inpatient visits each year.

To identify the hospital’s opportunities for improvement, Discern reviewed the hospital’s readmissions data, including rates of readmission by site of discharge and discharging diagnosis. Discern conducted interviews with hospital staff and the hospital’s post-acute care partners to identify opportunities for improvement in care processes. Discern also participated in cross- disciplinary case review meetings in which hospital staff identified the root causes of readmission for a sample of recently readmitted patients.

Discern synthesized its findings into a set of recommendations, designed to target the hospital’s leading opportunities to reduce unplanned readmissions, and presented recommendations to the Holy Cross executive team. Recommendations included improvements in general care processes and the creation of targeted interventions for specific populations with high rates of readmission. Several of Discern’s recommendations have been embraced by the hospital’s leadership and are in the process of being implemented.
