Discern Health is excited to announce the promotion of three Discern Project Analysts—Tess Donckels, Palak Patel, and Rachael Peroutky—to Project Managers. They have each demonstrated the ability to manage complex projects and develop creative approaches and solutions that help our clients succeed in the value-based care environment.
As a Project Manager, Tess Donckels contributes to quantitative and qualitative health care projects, including measure development to improve quality of care in chronic disease. In addition, Tess’s work includes supporting the creation of novel population health products that close gaps in care and reduce time to diagnosis.
Palak Patel leads projects that promote the development of a high-performance health system. Her work includes identifying opportunities to improve patient-reported measures for oncology, reviewing rulemaking documents to help clients understand value-based payment programs, and supporting quality initiatives to inform strategy development.
Rachael Peroutky serves as the primary researcher for measure development and quality improvement projects related to diabetes, cardiovascular disease, chronic kidney disease, and heart failure. In addition, she supports the development, drafting, and graphic design of strategic reports for multiple clients.
